 We offer Care Management Services in addition to our Care Giver Services to ensure we can provided unparralleled        

 quality care. Our Care Managers will oversee your care, manage the care giving staff, and offer suggestions to increase your quality of life. We also offer free in-home consulations in order to assess your needs and ensure we have matched your care giver to your needs and personal preferences.

 Personalized Care Mangament Plan

 

• Our RN and Care Managers will work together to generate a Care Plan

       that is tailored to your personal needs. 

 

 

 Recommended Activities

 

• Recommend daily activities such as plays, festivals,

     community events, and disease specific support groups. 

 

 

 Coordinated Care

 

• M​anage all care givers and ensure they are meeting your standards as well as CareStat's standards.

 

• Coordinate home needs including housekeeping, meal preperation, transportation, and medication reminders. 

 

• Work with your physicians to ensure you have the most comprehensive care possible. 

 

 

 Home Safety Inspections

 

• ​Assess your home every visit to ensure accessibility and safety using a Home Safety Inspection Check List.

 

• Make recommedations to help improve the safety and accessibility of your home. 

 

 

 Fall Risk Assessment

 

• Assess your condition to determine your risk for falls and make suggestions to prevent future falls. 

 

 

 Health Promotion

 

• Work together to find ways to increase your quality of life.

 

• Help with smoking cessation, diet, exercise, and stress management. 

 

 

 Family Involvement

 

• Keep family informed and involved, whether they are right down the road or across the country.
